Process for contracting with Bankruptcy VP

1. Download the Client Intake Forms. These forms provide for a complete financial situation and income history of your client, including the information required to complete the Means Test and Statement of Financial Affairs.

If your client owns a business, home based or otherwise, or has an income from partial ownership of a business, please make sure he receives the Client Intake Forms -Business in addition to the Consumer package.

Print out the instructions for completing the forms and make sure it is provided to the client with the Client Intake Forms package (s).

The packages can be emailed to your client via email in a PDF file, mailed or they can pick them up. If you provide us with an email address or address, Bankruptcy VP will send the package (s) to the client for you.

2. When the client returns the completed package to you, please make copies of any original documents the client has provided. This will always include a copy of their driver’s
license, vehicle titles, most recent pay stubs, court served papers, previous bankruptcies, wage garnishments, repossessions, divorce settlements and child support/alimony orders.

3. You can then send a copy of the information to Bankruptcy VP via fax, email or overnight courier.

4. Bankruptcy VP will draft the preliminary bankruptcy petition, including the Means Test, prepare a list of areas where addition clarification/clarification is required to make sure the petition is completed properly. This step can be eliminated if you give approval for Bankruptcy VP to contact the client to obtain the information.

5. When additional information is obtained, either from you, your office or the client, Bankruptcy VP will complete the petition, convert it to a PDF file and forward it to you
for a final review prior to filing it.

6. At your request, Bankruptcy VP will file the petition electronically in districts where the technology is available.
